2.Do you have chest pain, bone/joint
pain, high blood pressure, or high cholesterol?
3.Do you have Sickle Cell Trait
(SCT)?
4.Have you had a change in medical
status since your last PHA?
5.Are you ill today or aware of any
condition preventing participation?
6.Reservists (if applicable): Are you on active duty
orders or in drill status?
